Nalvee 10mg contains dydrogesterone, a type of medicine that works in a similar way to the natural female hormone progesterone. It is used as part of hormone replacement therapy (HRT) to support an oestrogen treatment in women going through natural or surgical menopause who still have a womb. Adding dydrogesterone helps protect the lining of the womb from the effects of oestrogen. Nalvee is not suitable for men.

What should I do if I experience side effects of Nalvee?

Stop taking Nalvee and contact your doctor straight away if you develop a migraine-like or unusually severe headache, or if your vision is suddenly affected. Also contact your doctor if you notice signs of a blood clot, such as painful swelling of a leg, sudden chest pain or breathlessness. Report any changes in your breasts to your doctor. You can also report suspected side effects to the MHRA Yellow Card Scheme.

Who should not take Nalvee?

Do not take Nalvee if you are allergic to dydrogesterone or any other ingredient. You should not take it if you have or have had a tumour that grows in response to progestogen (such as a meningioma), or if you have unexplained vaginal bleeding. It is also not suitable if you have severe liver disease, certain bile pigment disorders, previous or current liver tumours, or a history of blood clots in the veins (thrombophlebitis or thromboembolic disease).

What precautions should I take?

Tell your doctor before starting Nalvee if you have or have had porphyria, depression, abnormal liver function, or jaundice and itching caused by bile problems. Your doctor will need to monitor you closely. Before starting HRT, your doctor will review your full medical and family history, and may recommend regular check-ups, including breast examinations. Report any breast changes to your doctor. If you develop high blood pressure during treatment, this should be discussed with your doctor.

Can I take Nalvee during pregnancy or while breastfeeding?

Nalvee 10mg is not authorised for use during pregnancy. You should not take Nalvee while breastfeeding, as progestogens and their breakdown products can pass into breast milk in small amounts, and it is not known if this could affect the baby. If you think you may be pregnant or are planning to breastfeed, speak to your doctor before taking this medicine.

What should I do if I miss a dose?

If you forget to take a dose of Nalvee, take it as soon as you remember on the same day. If you do not remember until the next day, skip the missed dose and continue with your usual schedule. Do not take a double dose to make up for a forgotten one. If you are unsure what to do, speak to your doctor or pharmacist.

How should I store Nalvee?

Nalvee does not need any special storage conditions. Keep the tablets in their original blister pack and out of the sight and reach of children. Do not use Nalvee after the expiry date printed on the packaging.

How do I dispose of Nalvee?

Do not throw Nalvee tablets away with household waste or flush them down the toilet. Dydrogesterone may pose a risk to the aquatic environment, so any unused or out-of-date medicine should be returned to your pharmacy for safe disposal. This helps protect the environment.

Can Nalvee affect my ability to drive?

Nalvee has a minor effect on the ability to drive and use machines. Dydrogesterone may rarely cause mild drowsiness or dizziness, particularly in the first few hours after taking a tablet. If you are affected, do not drive or operate machines until you feel back to normal. Take extra care until you know how Nalvee affects you.

Does Nalvee increase the risk of breast cancer?

There is evidence that combined HRT with oestrogen and a progestogen, such as dydrogesterone, increases the risk of breast cancer. This risk becomes apparent after about 3 years of use and depends on how long you take HRT. After stopping treatment, the increased risk reduces over time. HRT can also make breasts look denser on mammograms, which may make it harder to detect breast changes. Tell your doctor about any breast changes.

Does Nalvee increase the risk of blood clots?

HRT is linked with a higher risk of blood clots in the veins, such as deep vein thrombosis or pulmonary embolism. The risk is highest during the first year of HRT use. Risk factors include older age, major surgery, long periods of being immobile, obesity, and certain inherited clotting conditions. If you are due to have surgery or will be immobile for a long time, your doctor may stop HRT 4 to 6 weeks beforehand. Contact your doctor straight away if you notice painful swelling in a leg, sudden chest pain or breathlessness.

Directions

Take Nalvee by mouth, swallowing the tablet whole. The dose and schedule depend on the type of HRT your doctor has prescribed. With continuous sequential HRT, you take oestrogen every day and add 10mg of dydrogesterone during the last 14 days of each 28-day cycle. With cyclical HRT, oestrogen is usually taken for 21 days followed by 7 days off, with 10mg of dydrogesterone added during the last 12 to 14 days of oestrogen use. Depending on your response, your doctor may increase the daily dose to 20mg. If you take one tablet a day, take it at the same time each day. If you take two tablets a day, take one in the morning and one in the evening.

Side Effects

Common side effects include headache, migraine, nausea, menstrual changes (such as irregular or heavy bleeding) and breast pain or tenderness. Less common side effects include depressed mood, dizziness, vomiting and allergic skin reactions such as rash, itching or hives. Rare side effects include drowsiness, swelling of the breast, fluid retention, weight gain and changes in liver function. Please read the patient information leaflet for a full list of side effects.

How it works

Nalvee contains dydrogesterone, a progestogen that acts like the body’s natural hormone progesterone. When taken alongside oestrogen as part of HRT, it causes the lining of the womb to change in a controlled way. This counteracts the effect of oestrogen on the womb lining and significantly reduces the increased risk of overgrowth or cancer of the womb lining that oestrogen alone can cause.

Ingredients

The active ingredient is dydrogesterone 10mg. The other ingredients are: lactose monohydrate, hypromellose, maize starch, colloidal anhydrous silica, magnesium stearate, macrogol and titanium dioxide.

Warnings

Do not take Nalvee if any of the following apply to you:

  • You are allergic to dydrogesterone or any of the other ingredients
  • You have or are suspected to have a tumour that grows in response to progestogen (such as a meningioma)
  • You have unexplained vaginal bleeding”,”You have any condition that means you cannot take oestrogen (when Nalvee is used with an oestrogen)
  • You have severe acute or chronic liver disease, or a disorder that affects how your body processes bile pigments (such as Dubin-Johnson syndrome or Rotor syndrome)
  • You have or have had a liver tumour
  • You have thrombophlebitis (inflammation of a vein with a blood clot) or a thromboembolic disease (blood clot in the veins)
  • You have a known thrombophilia (an inherited tendency to develop blood clots)
  • You have rare hereditary problems of galactose intolerance, total lactase deficiency or glucose-galactose malabsorption

Talk to your doctor or pharmacist before taking Nalvee if any of the following apply to you:

  • You have or have had porphyria
  • You have or have had depression
  • You have abnormal liver function caused by liver disease
  • You have had jaundice or itching caused by problems with bile flow
  • You develop high blood pressure during treatment
  • You have a history of stroke (cerebral apoplexy), particularly if a higher dose is needed
  • You have a personal or family history of breast cancer or other hormone-related cancers
  • You have a first-degree relative who had a blood clot at a young age
  • You are due to have surgery or will be immobile for a long time, as HRT may need to be stopped beforehand
  • You are taking anticoagulant (blood-thinning) medicines
  • You have any other risk factors for blood clots, such as obesity, systemic lupus erythematosus (SLE), or cancer
  • You experience irregular or unexpected vaginal bleeding while taking Nalvee
