Overview

Estradot is a small, transparent patch that you stick to your skin. It contains estradiol, a form of oestrogen, and is used as hormone replacement therapy (HRT) for women who have gone through the menopause. It releases a steady amount of oestrogen through the skin into the body. Estradot is also licensed to help prevent osteoporosis in postmenopausal women at high risk of fractures who cannot take other bone-protecting medicines.

What should I do if I experience side effects of Estradot?

Stop using Estradot and contact your doctor straight away if you notice signs of a blood clot, such as a painful swollen leg, sudden chest pain, or shortness of breath. You should also stop and seek advice if you develop yellowing of the skin or eyes (jaundice), a significant rise in blood pressure, a new migraine-type headache, or if you become pregnant. If you develop swelling of the face, lips, or throat (angioedema), do not use Estradot again. For other side effects, speak to your doctor or pharmacist.

Who should not use Estradot?

Do not use Estradot if you have, or have ever had, breast cancer or another oestrogen-dependent cancer such as endometrial cancer. It is also not suitable if you have unexplained vaginal bleeding, untreated thickening of the womb lining, a current or past blood clot in the legs or lungs, a known clotting disorder, recent or active heart problems such as angina or a heart attack, active liver disease, porphyria, or an allergy to estradiol or any other ingredient in the patch.

What precautions should I take with Estradot?

Tell your doctor about your full medical history before starting Estradot. You may need closer monitoring if you have fibroids, endometriosis, high blood pressure, diabetes, liver problems, gallstones, migraines, asthma, epilepsy, lupus, or a family history of breast cancer or blood clots. Attend regular check-ups and breast screening appointments while on HRT. Check your breasts regularly and report any changes to your doctor. Your doctor should review whether you still need HRT at least once a year.

Can I use Estradot during pregnancy or while breastfeeding?

No. Estradot is not for use during pregnancy or while breastfeeding. If you become pregnant while using Estradot, remove the patch and contact your doctor immediately. HRT is not a contraceptive, so if there is any chance you could become pregnant, you will need to use a non-hormonal method such as condoms.

What should I do if I forget to apply a patch?

If you forget to change your patch, apply a new one as soon as you remember. Then go back to your usual schedule for the next patch change. Skipping or delaying patches can make irregular bleeding or spotting more likely. If you are unsure what to do, speak to your doctor or pharmacist.

What if I use too much Estradot?

Because Estradot is given through the skin, serious overdose is unlikely. The most common signs of too much oestrogen are breast tenderness and vaginal bleeding. If these happen, your doctor may lower your dose. The effects of an overdose can be reversed quickly by simply removing the patch. If you are worried, contact your doctor or pharmacist.

How should I store Estradot?

Keep Estradot in its original sachet and carton until you are ready to use it. Do not refrigerate or freeze the patches. Store them out of the sight and reach of children. Do not use a patch after the expiry date printed on the packaging.

How do I dispose of used Estradot patches?

After removing a used patch, fold it in half with the sticky side inwards so it cannot stick to anything else. Throw it away safely, out of the reach and sight of children. Any used or unused patches should be disposed of in line with local rules, or returned to your pharmacy, ideally in the original packaging. Do not flush patches down the toilet.

How quickly will Estradot start to work?

Relief of menopausal symptoms is generally seen within the first few weeks of treatment. If after three months your symptoms have not improved enough, your doctor may consider increasing the dose. If you get symptoms of too much oestrogen, such as breast tenderness, your doctor may lower the dose.

Does Estradot affect my ability to drive?

Estradot has no or very little effect on your ability to drive or use machinery. You should be able to carry on with your normal activities while using the patch.

Directions

Apply one Estradot patch to a clean, dry area of skin on your abdomen. Do not apply it to your breasts or near the waistline, where tight clothing may rub it off. Avoid skin that is oily, broken, or irritated. Press the patch firmly in place with the palm of your hand for about 10 seconds, especially around the edges. Replace the patch twice a week (every 3 to 4 days). Each time, choose a different site and leave at least a week before reusing the same area. Use the patch continuously as part of ongoing treatment. If you have a womb, your doctor will usually prescribe a progestagen to take alongside Estradot. Your doctor will start you on the lowest effective dose, and may adjust this after three months based on how well your symptoms are controlled. You can wear the patch while bathing or showering.

Side Effects

Possible side effects include:

  • Mild redness, itching or rash where the patch has been worn
  • Headache
  • Breast tension, pain, or enlargement
  • Period pain or changes to your periods, including irregular bleeding
  • Nausea, indigestion, diarrhoea, abdominal pain or bloating
  • Low mood, nervousness, or mood changes
  • Difficulty sleeping
  • Back pain
  • Swelling (fluid retention) and weight changes
  • Raised blood pressure

How it works

Estradot contains estradiol, which is the same as the oestrogen made naturally by the ovaries. After the menopause, the ovaries produce much less oestrogen, which leads to symptoms such as hot flushes and to bone thinning. The patch releases estradiol slowly through the skin into the bloodstream, topping up your oestrogen levels. This helps to relieve menopausal symptoms and slow the loss of bone that happens after menopause.

Ingredients

The active ingredient is estradiol (as hemihydrate) 0.78 mg per 5 cm² patch, releasing 50 micrograms of estradiol per 24 hours. The other ingredients are: acrylic adhesive, silicone adhesive, oleyl alcohol, dipropylene glycol, povidone (E1201), ethylene/vinyl acetate copolymer and vinylidene chloride/methyl acrylate copolymer laminate (backing layer), and fluoropolymer-coated polyester film (release liner).

Warnings

Do not use Estradot if any of the following apply to you:

  • You have, or have had, breast cancer, or it is suspected
  • You have, or have had, an oestrogen-dependent cancer such as cancer of the womb lining (endometrial cancer)
  • You have unexplained vaginal bleeding
  • You have untreated thickening of the womb lining (endometrial hyperplasia)
  • You have or have had a blood clot in a vein, such as in the legs (deep vein thrombosis) or lungs (pulmonary embolism)
  • You have a known blood clotting disorder, such as protein C, protein S, or antithrombin deficiency
  • You have or have recently had a disease caused by blood clots in the arteries, such as a heart attack, angina, or stroke
  • You have active liver disease, or a history of liver disease where liver function tests have not returned to normal
  • You are allergic to estradiol or any of the other ingredients in Estradot
  • You have porphyria (a rare blood disorder)

Talk to your doctor or pharmacist before using Estradot if any of the following apply to you:

  • You have fibroids in your womb or endometriosis
  • You have risk factors for blood clots, or a family history of clots at a young age
  • You have risk factors for oestrogen-dependent cancers, such as breast cancer in a close relative
  • You have high blood pressure
  • You have a liver disorder, such as a liver tumour
  • You have diabetes, with or without effects on your blood vessels
  • You have gallstones
  • You suffer from migraines or severe headaches
  • You have systemic lupus erythematosus (SLE)
  • You have a history of thickening of the womb lining (endometrial hyperplasia)
  • You have epilepsy
  • You have asthma
  • You have otosclerosis (a hearing disorder)
  • You have heart or kidney problems, as oestrogens may cause fluid retention
  • You have high levels of triglycerides (a type of fat) in your blood
  • You are due to have surgery or will be immobile for a long period — your doctor may advise stopping HRT 4 to 6 weeks beforehand
  • You have ever had a hypersensitivity reaction such as swelling of the face, lips, or throat (angioedema) while taking oestrogen
  • You develop sensitivity or a skin reaction to any part of the patch
